New patients must first call or email to reserve an open appointment time. You will then receive a confirmation email with an invitation to setup your online portal account. You cannot setup an account until you have been invited to the portal by our administrator. You will need to create a login the first time you use it. You can fill out the new patient intake forms via the portal, or if you prefer, you can download the necessary forms from the "Forms" page on our website.
Once your account is set up, click the button below to access our online patient portal. Once your portal account is active it's a great way to communicate with us, schedule appointments, and reference information on your treatment plan.
